MEDICARE PART B (Flu & COVID vaccines only - please provide a copy of any supplement plans also)
- Bring a COPY of your driver’s license and a COPY of your health insurance membership card – this is necessary to correctly bill insurance for your vaccination(s). Most vaccinations will be covered at 100%
- A completed consent form for each individual receiving a vaccination
- Children under the age of 18 must be accompanied by an adult
